Children's Single Point of Access Multi-Agency Notification Form

This form is used for contacting the Reading Children’s Single Point of Access (CSPoA) when you have concerns about a child or young person.

Due to security constraints, you will not be able to save this form and progress later. Please ensure you have as much of the detail needed (if known) to hand before starting. Details of information needed are on the main CSPoA page on our website.

You must obtain parental/carer consent to complete the online form, unless doing so puts the child/young person at risk of harm.

If you are worried about an immediate risk of significant harm to a child, call 999. It is essential that you also share your concerns with us by telephone and follow this up by completing this form within 24 hours.
